What goes on FAL Form 5
The crew list follows the boxes of IMO FAL Form 5 as amended by resolution FAL.14(46), in force since 1 January 2024:
- Boxes 1 to 5: name of ship, IMO number, call sign, voyage number, port and date of arrival or departure, flag State and last port of call.
- Boxes 6 to 17 for each crew member: number, family name, given names, rank or rating, nationality, date and place of birth, gender, and the nature, number, issuing State and expiry date of the identity document.
- Box 18: date and signature by the master, authorized agent or officer.
